What is the VIAL OF LIFE?

The VIAL OF LIFE kit (Lifesaving Information For Emergencies) is designed to speak for you or your loved ones when time is a critical factor. This important medical information form can assist First Responders in administering the proper medical treatment during an unexpected in-home injury or illness.

Is a VIAL OF LIFE Kit Right for You?

Do you have a chronic illness – or care for someone with a chronic illness? Do you care for someone who is elderly or disabled? If you answered yes, then a VIAL OF LIFE kit may be especially important for individuals in these situations.

How Does the VIAL OF LIFE Work?

  1. Fill out the VIAL OF LIFE Medical Information form thoroughly and legibly. You may want to ask your primary doctor to help you complete the form.
  2. Fold and put the completed form in the provided clear reclosable zip poly bag and put the red VIAL OF LIFE DECAL on the front of the bag.
  3. Use the supplied magnetic clip to place the bag on the front of your refrigerator door.
  4. Place the second red VIAL OF LIFE DECAL on the front of your main entry door at eye level. This alerts First Responders that you are a VIAL OF LIFE participant.
  5. Inform trusted family members, caregivers and others about your VIAL OF LIFE information.
  6. Regularly review and update your VIAL OF LIFE form to ensure it remains accurate and legible, especially after any significant changes in your health or medication.
